Keep in mind that at this time, Dailen is still an apprentice! Your tattoo may have some imperfections and need touch ups. By submitting this form, you certify that you are aware of this and you consent to be tattooed by an apprentice.

Describe your tattoo in as much detail as possible. Please remember to include body placement and a rough size estimate (preferably in inches or centimeters). If you're booking flash, describe which design to the best of your ability! *

What days of the week are you generally available? Please note: weekends fill up fast. You're much more likely to get an appointment in a reasonable timeframe if you can make a weekday work! *
